The Cost of Fear is a tale of Tsushima that becomes available during Act 3: Kill the Khan.
Synopsis[]
Jin Sakai encounters a man dying on the side of the road. He tells Jin that he has been poisoned along with the rest of his family, who retreated back to their home. Jin heads up the road to the family's cabin, but the man's wife refuses to let Jin in, believing him to be responsible for their poisoning due to his actions at Castle Shimura.
Jin decides to help the family anyway. He travels to a nearby Mongol outpost to gather food and medicine. When he returns however, he finds the house set ablaze by a group of Mongol soldiers. Jin defeats the enemies and finds the wife gravely injured by an arrow. She tells Jin that the rest of her family is already dead and that they might have been saved had she let him in. The woman then dies, and Jin reflects on the importance of trusting one another.

I thought I heard someone coughing and crying out for help. I should look into it.
I found a man dying in the snow of Jogaku. Not from the cold, but from poison. Likely the same poison I put in the hands of our enemy. As he died, the man said his family was also poisoned at their nearby home. I can't let them suffer the same fate.
